Model release form?

I'm now starting to shoot models who aren't just my personal friends so I guess it would be in my best interests to have them sign a model release form. Do any of you have a generic form you could share with me?

I only plan to use the photos on my business and personal websites but would like the option to sell any of the photos to 3d parties if the opportunity would arise.

Well I did do that and I spliced together a model release form and a liability release form. I was still curious if anyone had any specific recomendations though. Here's what I've come up with so far:

The Photographer(s) and licensees or assignees may have unrestricted use of these for whatever purpose, including advertising, with any reasonable retouching or alteration.

I agree that the above mentioned photographs and any reproductions shall be deemed to represent an imaginary person, and further agree that the Photographer(s) or any person authorized by or acting on his or her behalf may use the above mentioned photographs or any reproductions of them for any advertising purposes or for the purpose of illustrating any wording, and agree that no such wording shall be considered to be attributed to me personally unless my full name is used.

Provided my full name is not mentioned in connection with any other statement or wording which may be attributed to me personally, I undertake not to prosecute or to institute proceedings, claims or demands against either the Photographer(s) or his or her agents in respect of any usage of the above mentioned photographs. I have read this model release form carefully and fully understand its meanings and implications.
I, do for myself (and for or on behalf of my child participant, if said child is not of age or older) do hereby release, forever discharge and agree to hold harmless the above mentioned photographer(s) from any and all liability, claims or demands for personal injury, sickness or death, as well as property damages and expenses, of any nature whatsoever which may be incurred by the undersigned and/or the child participant that occur while said is participating in the above activity.
Furthermore, I (and for or on behalf of my child participant, if under the age of 21 years) hereby assume all risk of personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved therein. The undersigned further hereby agrees to hold harmless and indemnify said organization(s), its directors, employees and agents, for any liability sustained by said photographer(s) as the result of negligent, willful or intentional acts of said participant, including expenses incurred attendant thereto.
