I taking a stab that your using filters which fit into a holder (Cokin) and the holder in one is square and the other its at an angle, so only affecting one corner. It happened to me a long time ago on a film camera and took me a while to figure out what was happening.

I was rotating the polariser and the holder rotated with it.
