I really like the way you caught the expressions on the faces and the lighting really brings out the expressions.
One very minor nit pick is that a little more space nest to the lady's arm would help here. This is very minor nit pick.

Sorry to hear about that. I can really closely relate to what you are going through. My son survived a near fatal head-on highway collision with diffuse axonal injury and bleeding in the frontal lobe.
I hope you have the rehabilitation and other professionals in place to work with your brother. There are a lot of great resources out there.

I like the use of b&w here as the colors would only distract from the 'point' of the picture.
The minor nitpick Roger points out is correct so if you cropped that space out you could put it back in.

My only crib here is the background; very busy and cluttered. The teacher and learner deserve our full attention without any distraction. So my suggestion would be to select the background as best you can and add a little guasian blur.
But the faces are packed with emotion (especially your brothers) and makes me want to stay looking at them and imagining what they are going through. Well done for taking this shot and showing it to us.
I wish your brother well in his efforts and hope he has a full recovery.

Pez, you really have some beautiful photographs, and this one is no exception. I might agree that the background is a bit cluttered, if it weren't for the fact that everything in the back seems to basically be the same "color". If there were more dark objects interspersed, then there might be an issue. The expressions are very telling. Nicely done.
Frog – Thanks for the feedback. I shot this pretty much wide open but being a wide angle it would capture more of depth.
Overbeyond – I could play with it a little but sometimes post processing can feel unnatural. As you noted I was after the expressions.

Ronnoco – I hope all is well with your son and his recovery. Recovery with a head injury can be a long process. My thoughts and prayers are with you and your son.

Without sounding trite or over spiritual; there is grace in the healing for my brother.
This shot really speaks volumes. I've no critique to add, just wanted to say that having watched a dear old friend go through much the same after a devastating stroke this really touched me. You've captured the feelings involved beautifully.
