Selling a CC-protected flickr photo / paperwork resource?

Hello. I apologize if this has already come up on this forum, but I can't seem to find it anywhere. I have been contacted by a business that wants to purchase the rights to my CC-protected photo, which I have posted on flickr. The business that contacted me wants me to supply them w/ rights paperwork that simply states that I am giving them permission to use my photo in exchange for $xxx. Is there an easy/free resource which has this kind of legal paperwork, which I can download/use? This has been quite a challenge to find.

What you are looking for is officially called a "photo license agreement" and a google search of that term yeilded 5,880,000 results with a lot of free examples posted within the first page of results for various licensing arrangements.
