Contact AMEX first thing. This may be an identity theft issue as well since they had your card information!

Amex is doing a full investigation, I was told to expect a call from the FBI fraud unit. I told them if they do catch this person I will testify to send his/her ass to jail. Turns out that this has happened to a few people in this area.

Now, sign up with a fraud protection company. I use Debix but there are lots of others. These companies prevent anyone from opening or closing any accounts regardless of the information they possess. Even you! A special contact number is used and you must answer the secret question and approve any changes. Well worth the $60 a year. Saved my hide once.

Wow, sorry to hear about this. I do hope that you get everything worked out.
As far as where to ship it back to, give AmEx a call they should tell you where it was ordered from. Have you contacted Papal to close the account?

It happened to me - turns out it was my next door neighbor's daughter. She stole a credit card bill from my mailbox and was ordering shoes, a stereo, jewelry - you name it. She would have it shipped to my house and retreive it before I got home from my rural type mailbox on the street. Some of the packages got through, and I was confused as hell. I was shipping the stuff back to where it came, cancelling my credit card, etc. when a detective shows up one morning trying to arrest ME! Turns out some of the stuff was purchased on a lady's brand new credit card she had only used one time at a vet's office where this girl worked. They then tracked the phone calls from QVC as coming from the house next door to me and arrested her. This was a major mess. She charged over $1,000.00 on my credit card, just by getting my account number, knowing my name, etc.
